A Wearable Device That Uses Electric Fields to Treat Pancreatic Cancer

Why Optune Pax Matters

 

Locally advanced pancreatic cancer is one of the most difficult cancers to treat. It accounts for a significant portion of pancreatic cancer diagnoses, and treatment options have remained largely unchanged for decades. Standard care has relied primarily on chemotherapy, with limited alternatives for patients whose tumors cannot be surgically removed. Novocure, a global oncology company headquartered in Baar, Switzerland, developed Optune Pax as a fundamentally different approach. Rather than using drugs or radiation, the device delivers alternating electric fields directly into the tumor through the skin. The FDA approved Optune Pax as the first device of its kind for the treatment of adult patients with locally advanced pancreatic cancer.

 

The approval was based on results from a large clinical trial called PANOVA-3, an international, randomized study involving 571 patients. The trial evaluated Optune Pax used together with chemotherapy drugs gemcitabine and nab-paclitaxel, compared to patients receiving the same chemotherapy drugs alone. Results showed that patients treated with Optune Pax and chemotherapy demonstrated significant improvements in overall survival compared to those on chemotherapy alone. The findings were published in the Journal of Clinical Oncology and selected as a top presentation at a major oncology conference.

How This Wearable Cancer Treatment Device Works

 

Optune Pax uses a technology called Tumor Treating Fields, or TTFields. These are low-intensity, alternating electric fields that are tuned to a specific frequency matched to pancreatic cancer cells. When applied, the fields interfere with the process of cell division, called mitosis, that cancer cells rely on to multiply. By disrupting this process, TTFields can slow down or stop cancer cell division and may even destroy the cells. Because healthy cells divide at different rates and have different electrical properties than cancer cells, TTFields are designed to target tumor cells without significantly affecting surrounding healthy tissue. Preclinical data also suggest that TTFields may help activate the immune system to further fight the disease.

 

The device consists of three components. Transducer arrays, which are electrically insulated adhesive patches, are placed on the patient's abdomen over the tumor area. These arrays connect to a portable electric field generator that produces the TTFields at preset, unalterable parameters set by the manufacturer. The generator is carried in a specially designed bag. Patients wear the device while continuing their daily activities, receiving continuous treatment as they move, work, or rest. Optune Pax is used alongside chemotherapy, not as a replacement for it.

Exploring the Clinical Results of Optune Pax

 

The PANOVA-3 trial measured several outcomes beyond overall survival. Patients treated with Optune Pax and chemotherapy experienced six additional months before their pain progressed compared to those on chemotherapy alone. The trial also reported significant delays in the need for opioid pain medication among patients in the Optune Pax group. Across multiple quality-of-life measures, including digestive problems and fatigue, patients using the device demonstrated improvement in maintaining their daily functioning for a longer period.

 

The device was well tolerated in the trial. Adding Optune Pax to chemotherapy did not worsen the side effects typically associated with the chemotherapy drugs themselves. The most common device-related side effects were mild to moderate skin reactions beneath the transducer arrays, including irritation, redness, rash, and in some cases open sores. Patients may also experience warmth and tingling beneath the arrays during treatment. Who Can and Cannot Use Optune Pax

 

Optune Pax is indicated for adult patients with locally advanced pancreatic cancer and is used together with gemcitabine and nab-paclitaxel as a first-line treatment. It should only be used after the patient receives training from qualified medical personnel who have completed a training course provided by Novocure. Patients must use the specific components included in the Optune Pax Treatment Kit and should not substitute any parts.

 

The device is not suitable for everyone. Patients with electrical implants should not use Optune Pax because the interaction between the device and implanted electronics has not been tested and may cause the implant to malfunction. Patients with a known sensitivity to conductive gels, such as those used on ECG stickers, may experience increased skin reactions. The device should not be used during pregnancy, and women of childbearing potential must use birth control while on treatment. The transducer arrays must be kept dry, and patients with underlying skin conditions in the treatment area should discuss with their doctor whether this may interfere with treatment.

Where Tumor Treating Fields Fit in Cancer Treatment

 

TTFields technology is not limited to pancreatic cancer. Novocure has previously received FDA approval for TTFields devices in the treatment of glioblastoma, a type of brain cancer, and metastatic non-small cell lung cancer. Across all three approved indications, the company reports more than 4,400 active patients currently receiving TTFields therapy worldwide. Novocure generated $655 million in net revenues in its most recent full fiscal year, with patients treated across the United States, Germany, France, Japan, and other markets. The device also received CE Mark approval for pancreatic cancer, opening a regulatory pathway for European markets.

 

The broader significance of Optune Pax lies in what it represents as a treatment modality. Most cancer therapies work through chemical or biological mechanisms, using drugs that circulate through the body to target cancer cells. TTFields take a biophysical approach, using electric fields applied externally to physically interfere with tumor cell division at the site of the disease. Whether this approach expands to additional cancer types will depend on ongoing clinical trials and regulatory outcomes. For patients with locally advanced pancreatic cancer, Optune Pax provides a treatment option that did not exist before, used alongside existing chemotherapy to extend survival and maintain quality of life.
